Complete Your Questionnaire
The sooner, the better!
Make sure to complete and return your questionnaire ahead of time.
This questionnaire helps the experts with their research, so please be as detailed as possible, and return it to the library as soon as possible.
In Person
You can pick up a copy at the Adult Reference Desk, and return it once finished.
Online
You can also download questionnaire without visiting the Library. The completed questionnaire can be emailed to library@berlinpeck.org or printed and dropped off at the Adult Reference Desk.
